Buyer(s) hereby agrees to purchase and Breeder, Susan Davis, agrees to sell the dog described above, under the following terms and conditions:

Initial Health Exam: Buyer(s) have three days in which to take this puppy to the veterinarian of his/her choice at Buyer(s) expense, and will receive a full refund if he/she finds this puppy sick upon arrival. Proof of the examination must be provided to the breeder. To receive a full refund Buyer must contact Davis Doodles within the three day period. Davis Doodles reserves the right to have a second veterinarian opinion. Initial: __________

Health & Maintenance: This dog has been sold with the agreement that it is to be kept as a companion animal, not a dog to be kept out of doors. This dog will be well cared for, given adequate shade, water and housing during periods outside. This dog will not be allowed to run loose outside of owners property. He will be kept current on all immunizations and will receive adequate veterinary care as needed. His exercise will be restricted from excessive runs on concrete prior to age 15 months to ensure healthy bone development. Initial: __________

Buyer's Agreement: To help the Breeder in maintaining a breeding program that produces healthy, quality animals, the buyer agrees to provide information to the Breeder on the outcome of health tests, such as veterinary hip evaluation, eye examination by a veterinary ophthalmologist, any heart evaluations and any temperament issues that are of concern to the owners. Buyer agrees to return the dog to Seller, or allow Seller to re-home dog should buyer fail to be able to keep dog. Initial: __________

Spaying or Neutering: The Breeder does not believe in ESN because of the risks associated with completing the procedure too early. Instead, Buyer agrees to spay or neuter this dog by the time it is 7 months old. Buyer also agrees to send to the Breeder a copy of the veterinary certificate as proof that the procedure has been completed. Initial: __________

Breeder's Warranty: A Warranty for Hip Dysplasia is provided within two years of purchase of this dog. Buyer must provide seller with proof with documentation from PennHIP. Buyer will receive a replacement dog or a full refund of purchase if he/she has complied with the guidelines for protecting developing hips of the dog and if the dog is not seriously overweight. Buyer must promptly notify Seller before taking any action such as euthanasia, or this warranty will not be honored. Initial: __________

No Financial Responsibility: Other than the above limited guarantee, the Breeder assumes no financial responsibility once this dog has left her premises. Buyer(s) hereby agrees that failure to honor all conditions of this contract can result in the Breeder reclaiming the dog without compensation to the Buyer. Initial: __________

This agreement must by signed and RETURNED prior to this dog leaving her premises or warranty is void and breeder is released from all contractual obligations.
